Terms of Use

By checking the box, the participant and I agree to the following:

BEHAVIOUR:


• During Mentorship Sessions & Workshop time, the instructor and/or assistant(s) will supervise all participants under the age of 18 at all times.

• If you have a participant/child/children with special learning requirements, please talk to program staff when registering so we can make sure your child has what they need in order to have a wonderful experience.
• Behavior while participating in SSAC programs must at all times respect the workshop space, fellow participants, the instructors, mentors, and SSAC volunteers and team members.

   

  Zero Tolerance:
 Inappropriate behaviour, whether physical or verbal, will not be tolerated. Offenders will be asked to leave immediately.

  AUTHORIZATION:

  In registering the participant/child named in this form to participate in the Youth Mentorship Program at the South Simcoe Arts Council, graciously funded by the Ontario Trillium Foundation. I, the undersigned parent/guardian, hereby agree as follows:

  1. To permit myself/child to participate in the full range of the South Simcoe Arts Council's Youth Mentorship Program activities.

  2. I understand that some program activities may have an inherent risk factor and that all appropriate precautions will be taken for participant safety. I agree not to hold the South Simcoe Arts Council or any of its employees or volunteers responsible in the event of an injury to myself/child.

  I hereby waive South Simcoe Arts Council and its personnel from liability for any accidental injury and for any damage to or loss of the participant's person or property.

  3. I understand and agree to the South Simcoe Arts Council Program Policies as stated above.
